Research Services
Additional Information Required for Research Services

If you are requesting a Research Service please provide the following documents and information with your request:

		Copies of all pertinent birth, baptismal, marriage, death and/or burial certificate, obituaries, Wills, et cetera.  Do not send us your original documents.
		Descendants chart showing all known family members including birth, marriage, and death/burial dates and places, and clearly mark your line of descent;
		A list of sources you have already searched.  Please be as specific as possible.  For example, for a census, include the year, county, township/parish;  for Wills include the name of the court where found;
		If you have used microfilms or microfiche produced by a regional or national archive please include film numbers and record class numbers;
		If you have used microfilms or microfiche available from the Family History Library at Salt Lake City, Utah, please include the film or fiche call number (not your order form number);
		Specific instructions as to what you require.

If you are requesting consultation services or a research outline please include a statement outlining your research goals.
